This Policy explains when and why we collect personal information, how we use it, the conditions under which we may disclose it to others, and what choices you have. It relates to all our business activities.
Total Health Physiotherapy Ltd is committed to maintaining the trust and confidence of our clients, and we have strict procedures in place to protect all of their personal data.
In order to plan your safe and effective treatment, we do need to gather sensitive data relating to health matters pertinent to the provision of our services. Such data is provided by you and given with your consent. At your first appointment, we ask that you complete a Physiotherapy and Massage Consent Form. Information requested on this form includes details of your GP. Should further medical treatment be required, we will only pass your clinical details to your GP with your explicit consent.
We also gather your contact and email details. We use this information for several reasons. Primarily, this is to contact you regarding your appointment or with follow-up information (such as exercises or any additional information you have requested).
For current/active clients only, we will occasionally email you with practice news (such as dates of annual leave and availability).
We do not rent or trade email lists with other organisations and businesses, nor do we gather data on racial or ethnic origin, political opinions, religious or philosophical beliefs, trade union membership, sexual orientation, any genetic or biometric data, or criminal convictions.
Children
Our services may be directed to children under 16 years of age.
Any relevant information is only collected with the consent of the individual holding parental responsibility for that child.
If you learn that a child under 16 has provided us with personal information without your consent, please contact us.
Booking an Appointment
When you book an appointment with us, you create an account with our online appointment system, Click Book. In addition to your name, DoB, and email address, you will also complete a brief initial assessment form. This information will be stored on Click Book, together with your appointment history. Marketing Information
We use a third-party provider, MailChimp send information to our clients. As stated above, this is to keep in touch with our current and active clients only, sending occasional news about the practice when appropriate (e.g., Christmas holiday dates).
